Lexington, S.C. — The South Carolina Law Enforcement Division charged Charyada Onetder Davis, 38, with six counts of forgery and presenting a false claim for insurance payment, the agency said Tuesday.

SLED opened the case at the request of the South Carolina Department of Insurance. Arrest warrants say Davis submitted altered or fictitious medical bills and a work excuse from a former employer to Progressive Insurance Co., with documents totaling $11,763.06.

Investigators said Davis admitted the documents were fictitious and that she submitted them to obtain additional money. The claims were denied, according to the warrants.

Davis was booked into the Alvin S. Glenn Detention Center in Richland County. The South Carolina Department of Insurance will prosecute the case.

Local coverage of the SLED release said the forgery counts and the false-claim count were filed together after the Department of Insurance referred the Progressive paperwork for investigation.
